All library staff are welcome to use this form to share their thoughts, comments, concerns, ideas, questions, worries, brilliance, etc., about what we’re doing as an organization, how we’re doing it, why we’re doing it, etc.
This form is entirely anonymous. No one will know what you have to say unless you say who you are on your form. However, if you would like a direct reply you must include your name.
Completed forms will be forwarded to Robin Deadrick, the Libraries’ Human Resources Administrator, as the single designated collection point. Robin will manage the submissions, providing responses to questions from those staff who identified themselves and forwarding feedback as appropriate. Personal information will not be shared unless provided by you and explicitly requested that it be passed along.
